(2) HopeAllianz Inc provides an order form (Counseling Services, LifeSkills Coaching, DBT, Wellness Workshops, Bookstore), registration form (Forums, Resource Center, Newsletter), and online surveys may require users to give us contact information (e.g., name, address, telephone, e-mail address), unique identifiers (emergency contact, local crisis center), financial information (e.g., account/credit card numbers, income level), and demographic information 9e.g., date of birth/age, state, zip code). We use customers contact information from these forms to send the user information about HopeAllianz Inc services or products (Counseling Services, LifeSkills /coaching, DBT, Wellness Workshops). The customers contact information is also used to contact the user when necessary. Users may opt-out of receiving future mailings (see opt-out section below). Financial information that is collected is used to check the users qualifications, to bill the user for products or services, and for other purposes, Unique identifiers are used when an individual is in need of crisis intervention and a contact needs to be made with a family member, friend, or crisis center in their home area. Demographic and profile data is also collected at our website. We use this information to tailor the website to meet the needs of the user.

(3) HopeAllianz Inc provides Counseling Services, LifeSkills Coaching, DBT, and educational programs through this website. An order form is required including payment information and emergency contact. All information provided and e-mail correspondence is confidential with the exception of child abuse or neglect, vulnerable adult abuse or neglect, and in situations in which an individuals' safety or the safety of another may be at risk (this includes suicidal and homicidal ideations).

(4) HopeAllianz Inc will provide chat rooms, forums, message boards, and/or news groups in the future to users. A registration form will be required. Please remember that any information that is disclosed in these areas becomes public information and you need to exercise caution when deciding to disclose your personal information.

(5) HopeAllianz In provides links to other websites within the Resource Center, as well as referrals to community organizations. HopeAllianz Inc is not responsible for the privacy practices or the content of such websites or information received from these organizations.
